I had changed my name as part of my naturalization process and received the name change affidavit. However, the US passport office did not return my name change affidavit. Does any one know how I can get a copy of it. My naturalization is done in Boston and I took oath in Faneuil Hall. If any one have their affidavit, could you please tell if there is a court name or phone number on that affidavit so I can call the court and see if I can get a copy?
