N600 Application - Help please

We are naturalized US citizen parents of 17 year old son. Got citizenship in March 2009. My son has US passport and has been living with us all along.

Now that I am submitting N-600, appreciate any help with the following questions.

1. Part 3 - Question 5 ( Information about entry into United States)

Is it first time when my child entered into US (1998) or latest entry after travel to abroad ( 2008) ?

2. Do I need to fill up both Part 4 ( Information about father applying) and part 5 ( Information about mother applying) ? Or only one of the two.

3. Finally what documents I should submit
a) Birth Certificate copy
b) Green Card copy
c) Marriage Certificate copy
d) Passports copy of parents and child
e) Proof of residence

Do I need to submit all above ?

4) Finally anything related to child, like his school records ? If so what kind of records ?

Fill out the form completely so they can figure out the child's correct date of citizenship. They are asking for his greencard date and code. IF he was not in the U.S. at time of first parent's naturalization a date of re-entry may be determinative on his date of USC, IF NOT it is irrelevant.

As the form instructions tell you, most if not all, the evidence is probably already in his file except maybe for the parents' natz certs (send copies only).

3. For (d) submit copies of your naturalization certificates instead of (or in addition to) your passports
4. Yes, submit records (e.g. school, medical, his own driver's license) showing that he had the same address as you when you naturalized. Also submit a copy of the identifying pages of his US passport.
