mumbai consulate h1b stamping exp... & DS156 questions.. please help!!

i m going to india in the 2nd week of nov and i will be going to mumbai consulate for my first time h1b stamping (f1 to h1b). I have a couple of questions about the DS 156 forms...


DS 156

1) Question # 15 : Home Address-
which one will that be my current US home address or the one in india ?

2) Question # 29 : Have u ever been in the US ? If YES when and how long ?
do i have to mention my last date of entry in the US

3) Question # 35 : Has ur US visa ever been cancelled ?
In my case, i received the first F-1 visa from mumbai and it was valid only for 22 months.. and so i went to mexico and got another F-1 visa for 8 more months.. When they issued the second F-1 visa the consular officer put stamp bearing CWOP (cancelled without prejudice) So do i have to say YES or No for the cancellation question.. because i think my visa was never revoked but older one was cancelled just because they were giving me a newer one in the same category.

Also if anybody has any recent experiences regarding first time h1b stamping at mumbai consulate in india ... please post them also .. it always helps to be prepared...

1. You can use either, if you are expecting the visa back in indian address, and the indian address is the address in the passport, better give indian address.
2. Not necessary, yes and duration are perfect answers
3. No your visa was never cancelled. It was just a procedure mexican US consulates adopted to void old visas. Some bureaucratic procedures are stupid.

--> I guess you can mention your indian address

Question # 29 : Have u ever been in the US ?
-->Yes
If YES when and how long ?
---> Indicate the dates from the day you landed in US till the date you left US to India i.e.2week Nov '03-whatever day you are leaving the country
do i have to mention my last date of entry in the US.
--> Yes

Question # 35 : Has ur US visa ever been cancelled ?
--> No

I have gone thru this forum about documents required for first time h1b stamping ...

some folks have suggested taking W-2 forms for all the years of stay in US. Now my h1b was approved this past jun 03 and so i have not received any W-2 on h1b stay in US... so even then should i take W-2s for all my years in US on f1... I dont have all of them .. however i do have 2001 and 2002.

Regarding paystubs .. are recent two paystubs alright... my year todate earnings in paystub would highlight my genuineness...? I might have misplaced some of my paystubs...

photographs of inside and outside of the workplace.... would photos of my desk be alright ...?

mumbai consulate has put out a questionairre for all h1b applicants ... do u folks have any idea what kind of answers they require and to what extent should these answers be technical or should be something that a layman (consulate officer) may understand... the questionairre is attached here with
