moving clinic from one place to another but in same street?(NIW)

please help me
I moved my clinic from my original place to one right infront of same street and address changed like this: 1, east quartz street, Scotch town. AZ
changed to : 2, east quartz street, scotch town, AZ
so only that number 1, was changed to 2 with remaining address remained same. I will be finishing my 5 years NIW soon. I am very anxious for this fact that I totally unaware at the time I moved to inform or do something about it. is it too late now?
INS already has first one address in records(especially first 3 W's) not that second address
but my w2's of last two years and other documents reflects second address
after 3 years of my sponser contract, I have my own practice for remaining last 2 years with new address but same HPSA location and street but different clinic. (moved to neighboring office)
 
If address #2 is in the same census tract as address #1 and therefore in the same HPSA/MUA you shouldn't have a problem. Check the geography for both addresses on
www.census.gov
click on 'census 2000'
then click on 'find a street address'
 
Moving on the other side of a street can at times be a problem. Census tracts frequently follow streets, and if the new CT happens to have the local hospital in it you could get screwed.
