Moving 84 yr old Grandma from Canada to USA

Background:
Grandma is an 84 yr old recent widow and lifelong citizen of rural Canada with no family in Canada to care for her as she ages. She is still in good enough health to move to the US. All of her living family is in the US. She receives Canadian health insurance and her deceased spouse's pension.

Problem:
It is becoming difficult for family to get to Canada every time Grandma has an issue. Plane tickets run $1500 per person to get to her rural area, and travel takes an entire full day or two days just to get to her. We would like to move her closer and care for her here. We have no idea if this is possible at her age.

Additional Info:
The only family she has are one daughter, and two grandchildren. All are Canadian citizens with permanent legal US status (green cards) and have been living in the US for over 25 years, and have been permanent US residents for 25+ years. Both grandchildren are married to US citizens.

Questions:
1. Has anyone ever moved a relative over the age of 65 and found a way to "buy-in" to medicare or other health plan in the USA?

2. Does anyone know a good immigration lawyer we can consult with? We would prefer one in the USA and prefer one in the Midwest region (Indiana, Ohio, Michigan, Illinois, Kentucky area)

3. Any suggestions from people that have been in similar situations. We thought about her living here "part time" in an apartment - but as she ages a little more I am not sure she can make the trips back and forth. We are open to any and all suggestions - We have no idea what to do with the her situation as of right now.
