Missing Packet 4

Hi All,

We have received interview letter (Yellow) from the US Consulate in Chennai. The letter mentioned about the Enclosures (Encl: Packet 4) but there are no enclosures in the mail.

Seems like they are missing. Do I need to be concerned about this ?

We received the letter to our address in US.

You do not have to be concerned about this. You have received the only important document in the packet 4. If received at India this so called
PACKET contains blank copies of DS 230 forms, blank copy of Medical form and instruction sheet apart from the appointment letter.

When mailed to US address they only mail the 'Appointment letter'.

You are all set, get the DS 230 and Medical form from the net.

P4 by e-mail

Hi guys,

Thought i'd share my info, should it be of help to any of you.
My interview is at Chennai on Nov 16 - received the yellow interview letter (at US address) from chennai consulate, but have not yet received an actual P4 at india address. E-mailed chennai to enquire when to expect P4 - today got a reply saying they will send all info (P4) by e-mail - an hour later got their e-mail with a whole bunch of Docs.
Guess i dont have to worry about P4 anymore.

It is nice that the Consulate responded with this email and the attachments.

My co-brother is also attending his interview on Nov 16th. He also got just this one sheet at this Attorney's address. I have reassured him that it is all
he needs.

For the benefit of forum members can you list the other documents that they
sent as attachements to you?

P4 by e-mail

Hi all,

Most of the docs they sent are just std info on the docs you need to bring for the interview, which I am assuming by now most of you are already familiar with, and have acquired.

Here is the list of .doc files i received from chennai.

1. Interview letter
2. PDF of DS230
3. Letter listing common mistakes made while filling out DS230 applications (the same things you have already heard - complete all sections, dont use "NA" or "not applicable", only use "none" or "never", dont sign form)
4. Medical exam cover letter
5. Details relating to getting PC
6. SSN-Part II (this is a form you need to fill if you want to apply for new ssn card (by doing so your ss# will not change; SS office will just mail you a new card which will no longer have the H1-B associated job restriction)
7. Letter indicating list of docs to carry for EB based visas
8. Letter indicating list of docs to carry for family based visa
9. Letter stating that spouses and children (benificiaries) need to show sufficient edivence/proof of relationship to the petitioner
10. Letter listing all the doucument to be carried and the order in which they should be arranged, along with details of Visa Fees and Courier charges.


Information listed in #10 is as follows:

Arrange your documents from top to bottom in the order listed below and place them in your passport for submission when you come for your visa interview. Original affidavits will not be returned. If original certificates of your marriage, divorce, birth, military discharge etc., are desired, please include one photocopy.

OF-171- Visa appointment letter.
DS-230- Part I & II Visa application.
Marriage certificate and Wedding photos.
Birth certificate OR statement of non-availability of birth records & affidavit of birth
Evidence of financial support (Affidavit of support in Form I-864; recent 3 year U.S. Federal income tax return copies with W-2 statements; Job verification letter and Bank Statements from the Petitioner AND as well as any co-sponsor if required).
Two photographs must be an un-mounted full-face photo taken within the last six months. The photograph must be a full front view, taken facing the camera - no side or angled views are acceptable. The photograph should measure 2 inches x 2 inches (roughly 50 mm x 50 mm) with the head centered in the frame. The head (measured from the top of the hair to the bottom of the chin) should measure between 1 inch to 1 3/8 inches (28 mm and 35 mm) from the bottom of the photo. The photograph must have a white or off-white background.
Proof of relationship between beneficiary and the petitioner and with any other family members who are applying with you. – see attached.
Police Certificate – see attached.
Medical reports. Do not submit your X-ray unless requested to do so.

Visa Fees:
Bring a Demand Draft for the equivalent of $335 per applicant in Indian rupees made payable to ‘The American Consulate General, Chennai’, IF this fee has NOT previously been paid to the National Visa Center in the U.S.
The rate of exchange fluctuates often. Please call 2811-2060 for the appropriate rupee amount before obtaining your drafts. Bank exchange rates are not acceptable.

Courier Fees:
Blue Dart Courier will return all application materials and the applicant’s passport(s), including the visa, to the destination of the applicant’s choice after the visa has been approved. Courier charges are Rs. 300.00. Additionally, Rs. 50.00 are required for each immigrant visa and passport for insurance against loss/damage due to fire, natural calamities, theft and accidental damages while in transit.
Applicants are required to bring one demand draft, which should include both the courier fee of Rs. 300 and the insurance premium of Rs. 50 for each family member as explained above. This demand draft should be payable to Blue Dart Express Limited.

Note: We request that due to severe space constraints in our consular facilities, only the applicants are permitted into visa waiting room.

Raju, you mentioned your co-brother is having his interview at Chennai, the same day as I (Nov 16th). Perhaps i'll get to meet him - another fellow CP applicant. I am in EB-1 OR ; 8:00 am interview.

I am leaving for india on the 5th.
