Missing last name

My brother and his family are applying for a vistor visa and my brother has a missing last name in the passport.

His name: Raji Kumar
Wife: Kiran Katiyar
Daughter: Kriti Katiyar
Son: Rishi Katiyar


Now in his old passport, that had only one field for the name (there wasn't any seperate field for last name) has "Raji Kumar". When he had the passport renewed the new booklet had a seperate column for Last Name. But the First name has been entereed as "Raji Kumar" and last name has been left blank.

Now all his school certifictes etc have Raji Kumar BUT his buisness papers etc. have Raji Kumar Katiyar. Fortunately the passports of the family members hve his name correctly spelled out "Raji Kumar Katiyar"

So will this be a problem? The DS 156 on the web has the last name as mandatory, what should I fill in that?

I would ask him to get his passport amended by asking them to add his proper last name. It will be good to take care of this right now. This will prevent any other problems down the line.

Bottomline is make sure whatever is in the passport matches information in DS156 and DS157.
 
I don't think you can correct the passport name from "Raji Kumar" to "Raji Kumar Katiyar" without getting a new passport. However the name "Raji Kumar" written in one line can be corrected to have "Kumar" as last name by a procedure called 'Observation" which can be done at the passport office in a single day.
