Minimum time req to apply for H1-B extension before expiration

Hi Guys,
Is there a certain time before current H1-B expiration that we cannot apply for an extension?
I heard from someone that you cannot apply for an extension before 60 days to H1-B expiration?
Is that true?
If so, what is the impact/result if we do?
I sent mine through premium processing (current H1-B expires on Mar 31 04) so I can bring my spouse back with me when I visit India (since there have been cases where spouses' H4 was rejected if the H1-B was close to expiring). What are my options?

Or is this only the time limit for revalidation in the US (SL & DC)
can I go to canada 5 months before my current H1B visa expires and get the extension stamped.

No you can pick a future start off date. I think the limit is 180 days from expiration date.
The 60 day time limit is for revalidation- either here in US or in any US consulate.
 
60 Days for revalidation?

So, if I go for revalidation before 60 days to expiration, to a US consulate outside the US, will they deny the visa?
I plan to go to India in Late Jan 04 and should I wait till I am within 60 day window to go to revalidation?
Is there any other alternative?
I heard some one say that the 60 day revalidation window is only if we apply for revalidation with the US (St. Louis or D.C.), is that true?
