You can leave the middle name section blank since the instructions says to enter your information as listed on your passport. Same as above. If no family name aka last name aka surname is listed on your passport, you leave that section blank on the DV entry form. Unless only one name is listed on your passport in which case you list that name only under the family name section on your entry form.I also don't have a family name I want to know if I can leave the place blank

If you have only one name, it must be entered in the last/family name field.
