Medicals- vacinations

Can somebody please confirm the following list of vacinations needed ?
Which ones are mandatory ?
I am planning to get certificate from Dr in India. Is there any particular format ? Does it have to be from the Dr who administered the vacine or anybody else is ok ?

Call the CIS approved physicians in your area, they are in the best position to tell you exactly what you need. Call more than one, some are more stringent in their requirements than others when it comes to vaccination.

Why go through all this hassle. The vaccines you mentioned are available for $10. The main problem is chickenpox which is expensive and not in the list.
