Medical RFE response

My wife has received RFE on medical. I'm plannning to send RFE response on my own(not through lawyer). Can you suggest what needs to be sent to USCIS other than medical test results. Please share your experiences. Your help is greatly apreciated. Is there any template response letter that you can share?

I think the medical should be sealed by the examiner (MD) designated by USCIS, I might be wrong but please check into it before sending it in.

Thanks, sizzu. It seems you have plenty of time to respond the RFE. Nowadays CIS only gives 30 days to respond RFE, but you got 120 days (12 weeks). What a surprise, is it about TB skin test?

Yes. I was surprised too. Medical RFE is regarding vaccination.
