Medical Exam. questions?

where to get Supplement Form for I-693 Form (medican exam.)? Does the Doctor required to fill that form?

Also, there are 4 copies of I-693 form, does this mean the applicant can get the exact copy as the one sealed and sent to INS?

Usually doctor has the forms.

call him up to find out if he has forms. Some doctors charge $10 for each form.
If not you can always request from INS either through mail or in person.
mail will take atleast two weeks to get.
Of the four copies in the form, applicant will get a copy for his records.
 
Info for you..

If you go to any of the doctors mentioned in the following list, they should have the required forms. Also, INS will only accept physical examination done by one of the doctors on the list on this link website! They might not charge for the forms (if they do, you can always request forms from the INS or your lawyer.)

Make sure that they give you your copy of the examination- they are supposed to tear your copy off from the forms and give it to you and they put rest of the copies in a sealed envelope and give it to you! You are NOT supposed to open that envelope- you give it to you lawyer to mail it to the INS!
I believe some physicians seal all of the copies of that form, but they are NOT supposed to do that- legally they ARE supposed to give you YOUR copy of the exam as required by law!!
And YES- the applicant gets an exact copy of what goes to the INS! (I believe its the bottom copy from that form that you get back after your exam!)

(By the way- all these exams will take you almost a month to get done! Some TB test require you to wait for certain days before you go back again- and then the AIDS/blood test requires quite a few weeks to get the results! Hopefully you\'d get the exams started in time so that you don\'t waste too much time! You should start your medical exams atleast 6 weeks before you\'d be filing for your I-485. Medical exams are valid for upto one year- and if you don\'t file for your I-485 within that one year, then you have to do it again!)

We got results from doctor within 5 days - Of Course we tested negative for TB

But he did all the tests - Aids etc etc and gave us immunization shots

I got this done from a doctor on the list from Glendale, CA (Los Angeles area)

I am surprised when u say this will take a month
 
It took me over 3 weeks!

My results were negative as well, but it took me over three weeks from the day I went to get my test done! In fact I already had TB test done before that (did that when I travelled a while back and also got all my immunization shots back then- which saved time!)
Blood test/etc is what took the most time even though those tests were negative as well! Maybe times were longer in my case! Did it in Sunnyvale, CA (at Camino Medical Group). Maybe it took you less time because you went to a doctor rather than a clinic which has tons of immigration applicants coming in everyday!
 
DesiIn... 3 weeks is at least 2 weeks too many...

We got the TB test serum injected on our first visit and came back in a couple of days and got the required shots. During the first visit they drew blood and the results were back when we went to the doctor the second time. Luckily, this doctor had the tetanus shots in stock unlike many other doctors. Some doctors/clinics do not administer shots. You have to go elsewhere for that. That is why it is important to choose your immigration doctor very carefully. You do not want to spend a month wrapping up your medical exams when you have a lot of waiting to do further down the road to AOS.
I just downloaded the I693 form from the net and filled up as many copies as required. You do not have to wait for the INS to send you the form if your doctor does not have it. I am gonna try attaching the form to this post.
