do i have to complete the medical/immunizations before filing for I485? or do i just bring the i693 forms to the doctor regardless of when i485/i765/etc have been filed or not?

Yes
Medicals are valid for 15 months and all you need to take to the doctor is 693 forms, ID and a fat check
If your medicals are incomplete (as noted by the doctor) you will get a medical RFE later.
If you did not send your medicals your 485 app is liable to be rejected as incomplete or frivolous.
