Doctor put in "Had Chicken Pox as a child" though I never had Chicken Pox in my life. Is that okay? He says it always good to declare that I had Chicken Pox because that will prove that I am immune to it? Is this normal operating procedure? And he charged me $300 for thw whole thing.

Is this Okay for GC?

Also, he did not take any of my passport info! Does he not have to fill in the Passport No in I-693?
 
yes...That is normal..
See...the scenario is...
If u have chicken pox in childhood u will Never get it in the life..(and typically every One from India will get get chiken pox)..
And some how if u did not get chicken pox in childhood..the doctor usually gives a vaccine or medicine or whatever...

In my case...I went this doctor who asked me the same question..and I said no..and he said check with ur parens cos everyone from my country will get it...and i checked and found it to be true...

Basically, We need to send sealed I-693 form to INS.

if we did not take nessary immunisation, then doctor would give some and look for reactions and sign I-693 form.

However, some one (either you or doctor) should fill all the necessary fields in that form.

In My case, i filled all my information ... nurse gave me MMR.
take blood for AIDS test, TB skin test. doctor just put formality questions ...

After 3 days, i called them for results ... they said it's ready ....
They hand over sealed cover ... i asked them what they have in that cover.

They gave me photo copy of I-693 ... just doctor sign and checked options,which tells we are up to date for immunization and no AIDS and no siphilius ....

I paid 190 for each person.

You have to go through an INS certified medical doctor. They charge from US$ 200 to 300 per person. I got 5 injection for this exam.
You will get a sealed medical report for each person(don't open'). Send that to lawyer. Lawyer will file your 485 case using that report.
 
$125 @ NYC, shop around!

If you never had chickepox, it may worth the trouble have a vaccine, $70 I heard. It may work with all kinds pox, you know what I mean ;)

hELLO

Make sure ur doctor include Supplement Form to I-693 ( Immunization form). I heard that one guy got RFE for this form.
This form asks for Passport #.

chicken pox vaccine

In my medical test, I told the doc that I had chicken pox and I am getting the report faxed. When I went to get my report and showed him my report, he said that he wont accept it.

I told him that I need to apply 485 asap since he said I have to wait for a month before he can give me the vaccine. He ended up putting insufficient time in the supplement.

My question is whether this is going to result in an RFE ? If so, should I get the vaccine and send the update to INS ? Anybody in this kind of case please help !

Also, at the bottom he metioned that he recommends approval. I was negative for all the tests like TB etc.

yes even the Doctor I went to refused to fill the passport #

He said they have never filled out passport #s for anyone so far and said it will not be a problem( I did argue a lot to get him to put in my passport #, but couldn't suceed). I could not understand why he refused to fill the passport #.
He charged 220$( 140 + 80$ for varicella vaccine).

I am hoping the blank passport # field does not become a problem.

Has this happened to anyone?
