Maiden name or after marriage last name...

I am trying to take NOA birth certificate for my mother.

In the following format where "YourName" is there, I am supposed to write my mothers name. Right?

My question is what should be the last name: maiden name or after marriage family name

Non-availability Certificate: Issued under Registration of Births and Deaths Act, 1969.

This is to certify that a search has been made on the request of <your name>, son of <father’s name> in the Municipal registration record for the year(s) 1965-1975 relating for Mumbai, Maharashtra and found that the birth record of <your name>, son of <father’s name> is not registered/available.

--------------------what is NOA? do you mean you have to get Non availability of birth certificate for your mother from Mumbai Municipality ?

Non-availability of birth will be having your mother’s maiden name on it and once she got married then she changed her maiden name to married name. For married name she may be having mirage certificate.
 
There are two places that has "YourName",

one is ....search has been made on the request of <your name>, ---> 1
and the other is ....the birth record of <your name>----- 2

She will be requesting now and her current last name is the one she got after marriage.

So which one of the following options should we use:

1. Use maiden name in Position 1 as well position 2 for last name
2. Use after marriage name in Position 1 and maiden name in Position 2 for last name

If the last name before marriage i.e. maiden name was XXX and the last name after marriage is YYY then which one of the following certificate is correct.

Mohanpc - You are assuming that everyone will understand what you have in your mind. Be more precise of what you are looking for.
 
he is just asking if he should use the "maiden name" or "married name" on the non-availability certificate.

so you are trying to get non-availability certificate for YOU. if you are getting
non-availability certificate for YOU then it should have your maiden name on it.

if you are trying to get non-availability certificate for your MOTHER then it should be your mothers maiden name on it.

from Mumbai Municipality you will get non-availability certificate in Marathi language. once you get non-availability certificate in Marathi language then go to Notrary Public and let him translate the non-availability certificate in English on stamp paper and Notary will sign and stamp the seal.
