Lost Visa

I lost my passport and re validated visa while here in US what are the necessary steps to get the visa again?

Please help.

what about I94?
get the new passport. File for revalidation with St.Louis with the copy of police complaint, copy of old passport with visa and h1 approval again.

I lost mine too in 09/2000. What I was did… I went to Indian consulate in San Francisco.
They told me to get a police compliant, I went to near police station and reported that I lost my passport; cop gave me a small receipt. I included this with lost passport application. It took three weeks to get it.

You may have to include:
Employer letter, copy of passport if available, notary, driving license
