I have lost my passport, visa and I-94. I would like to know where would I get information to apply for new I-94 and how much time does it take to get the duplicated I-94.
You'll have to fill up the form I-102. You could download the form available at the following location http://www.ins.gov/graphics/formsfee/forms/i-102.htm
and you'll have to send a MO/Cashier's check for $100.00 to NSC (in my case it was nebraska. It could be different for or you.. or I'm not sure whether you could send it to any service center).
A copy of the following items if you have would help.
1. copy of old I-94
2. copy of your I-797
3. copy of your (lost) passport (all pages).
If you don't have a copy, you have to recollect the date of entry and validity and the port of entry and mention it in the form I-102.
Elsewhere from the forum I guess it's taking about a month or 2 for getting a new I-94.
