Lost I-94

Hi friends,
I am on H1B. Unfortunately I lost my I-94 and SSN card but I have photo copies of both..
Whom do I contact and what should I do? Please help me :)
I am working in Dallas and my port of entry was Baltimore (washington) and my employer is in Philly..

I got my SSN in Fairfax.

I had a similar situation when I was on F1. I had lost my I-94. I approached International Students Office at my school. They told me to fill up a form and charged $85 as fee. It was sent to INS service center in TX. I got my I-94 after a month or so. I would suggest you to talk to a INS office nearest to you.

--- you can apply for I-94 fill up the form I-102 read instructions and mail it to service center with fee
--- you can get new SSN card go to SSA office

***( I would request to change the SSN# becase on I-94 your name and DOB was written and the other person also has got SSN#, take the precaution)
