Lost H-4 approval notice

Hi,

Lost my wife's first h-4 Approval notice. Will it be ok to submit remaining three h-4 approvals/extensions notices?

Is there a way to get the duplicate h-4 approval notice?

if she has valid I-94 then submit the copy of I-94 (front and back) approval notices are not needed.

She has the lastest 1-94...but not the old 1-94's. Also my wife's passport has the old Visa which is based on the first h4 approval...will this work?

------that is OK file her I-485 with Copy of her valid I-94 (front and back)
