Licensure for H1B processing

Can any one tell me if a State Medical Licensure is a requirement for obtaining a LCA(for H1B)for the purpose of starting a Physician job in a particular state? Or is it enough if you just pass USMLE step3 examination. If you need a valid Medical Licensure, do you need to possess one from that particular state that you intend to work in or can you substitute it with a Licensure from a different state? Do you need Licensure for LCA or do you need to furnish it only when filing for INS for a H1B1? Also can you file for LCA or INS saying that you passed your USMLE step 3 Examination but are awaiting on the Medical Licensure. Thank you in advance.
 
Thank you very much for your reply,kidsdoc. Can you also tell me if you may use the licensure from any state or do you need to have license from that particular state that you intend to practice?
 
but for applying the LCA,YOU NEED TO HAVE A PERMANENT STATE LICENSURE AND FOR THAT YOU NEED USMLE STEP 3 CLEARED AND FINISH 2/3 YRS OF RESIDENCY TRAINING
