I have downloaded the fax form from one of the threads on the forum which is used for fax inquiry.

1. What form do I use to make level II inquiry? What is the fax no for level II inquiry?

2. What is the fax no for level one inquiry?

My case looks same as yours.

I did my 2nd FP, early Jan'2004. My case was touched on 04/07/2004.

My 3rd EAD has been approved in 30 days.

I am only seeing one form in the document on page 38.

1. Is there a separate one for level II inquiry? If not, what is different on the form for level I and level II inquiry?

2. Also, what is the fax number for level I and level II. The form does not mention fax numbers related to inquiry levels.
 
I used the same form for both inquiries - no response, though.
Fax numbers are on the right-hand side

Division I (level 1) - 949 389 3055
Division XII (level 2) - 949 389 3486

There is an e-mail for level 2, but it's not working.
