I approached our local senator some time back. What I did is I downloaded their case sheet. Filled up with my details and attached separate sheet for explaining my problem (because case sheet space is not good enough to wrie my entire story).
I believe it is the same case for every senator.
I think I used senators.org to download this case sheet.
