PCC is required from all the countries u lived in for over 6 months sice attaining age 16. US is an exception, u don;t need PCC from US.
in the case of most of us, as we atayed only in india and US, PCC from Indian authorities (RPO or Consulate) works.
so probably u need one from RPO or indian consulate and one from ur current country if u stayed there over 6 months.
2. DS230 asks for a couple of addresses...u can give current address as middle east, other address is for where u need to receive ur GC Card..
come up with more specifics like which item or point #, i\'m not gonna look the form and then write back here.
3. along with affidavit from mother (preferably both parents) and school cert, try to get non-availability cert from municipal corporation(format given in immihelp sample documents). it\'s not a bigge to get it in India
4.i\'m not sure of specifics of what CDC recommends but i\'d say within 2 weeks prior to interview.
