LCA and H1B transfer

Please clarify -

1) Does LCA and H1B-transfer-application get filed together OR
having an approved LCA a pre-requisite for filing H1B transfer ?

2) Is it allowed to join the new co. (H1B transfer case) after that
co. has filed for H1-transfer OR after the co. has received the
Receipt Notice for H1-transfer ?

3) How long does it take, approx., to get receipt of H1-transfer ?

4) How long does it take, approx., to get LCA approved ?

3) How long does it take, approx., to get receipt of H1-transfer ? 2 weeks

4) How long does it take, approx., to get LCA approved ? LCA is through Fax. Almost a day.
