Latest DS-157 form

Is there a latest version of the DS-157 form available. The current form expires on 07/31/2005 which is available on the Department of State website. Am not sure if we can use this r not?? Would appreciate ur replies as I have my visa interview in a couple of weeks time. Thanks!

Having problem adding an applicant to the existing application

Hi Anyone,

I completed DS-156 and DS-157 forms successfully for my father through VFS site. After completing my fathers details, I clicked on “Add applicant” button to add my mom for this application.

I get this when I try to add my mom as an applicant:

"Either your receipt is not activated or the name of the person to which the receipt belongs is not correct."

Chennai office confirmed that my moms receipt is activated since it was issued on Oct 4th, 2005 and it takes only 2 days to get activated

If anyone out there happened to successfully make appointments for both their parents, your help with detailed steps on this is greatly appreciated.
