Last hour feedback in (I-130, G-325A, I-864, I-485, I-765) forms

I request to experience person to look my table of content. Please let me suggest if I am missing anything or providing extra documents. I have one question in my mind that I am going to send 2 copies (some of 3 copies) of my non US passport, I-94 and birth certificate, marriage certificate in Form I-485, Form I-130, and Form I-765. I don’t know it’s right or not? Should I only attach my non US passport, I-94 and birth certificate only in I-485? I got married in USA and right now I am in F1 Visa. This week i am going to send (I-130, G-325A, I-864, I-485, I-765) forms . hoping for yours feedback.

1. Check: Payment in the amount of $1,070 payable to U.S. Department of Homeland (Biometrics fee included).
2. Form G-1145 E-Notification of application/petition acceptance.
3. Completely filled out and signed I-485 form.
4. G-325A, Biographic Information Sheet.
5. Form I-693, Report of Medical Examination and Vaccination Report.
6. Two (2) passport size pictures.
7. Copy of birth certificate.
8. Copy of passport pages, with nonimmigrant F1 1visa.
9. Copy of I-94 card (front and back).
10. Copy of marriage certificate.
11. Copy of school I-20:
1) Lake Superior College
2) University of Minnesota Duluth

B- Form I-130: Petition for Alien Relative package
1) Check: Payment in the amount of $420 payable to U.S. Department of Homeland (Biometrics fee included).
2) Form G-1145 E-Notification of application/petition acceptance.
3) Completely filled out and signed Form I-130.
4) Copy of birth certificate of applicant.
5) Copy of birth certificate of beneficially
6) Copy of marriage certificate.
7) Copy of passport pages, with nonimmigrant F1 1visa.
8) Copy of I-94 card (front and back).
9) 1 photo each of applicant and beneficiary.
10) Completed Forms G-325A for applicant and beneficiary (attachments included).
11) Evidence of Bona fide Marriage:
a. Copy of Joint Bank account statements.
b. Copy of Transfer on Death (TOD) Registration.
c. Copy of apartment lease paper.
d. Copy of joint phone bill statements.
e. Copy of joint electricity bills.
f. Copy of car insurance.
g. Selection of pictures that document relationship and marriage.
h. Copy of pre-marital education certificate.

C- Form I-765: Application for Employment Authorization:
1) Form G-1145, E-Notification of Application/Petition Acceptance.
2) A copy of Form I-94, Arrival-Departure Record (front and back).
3) A copy of passport showing picture, name, date of birth.
4) A copy of visa page.
5) Two copy of passport size pictures.
6) Copy of marriage certificate.

D- Form I-864: Affidavit of Support:
1) Copy of IRS Tax Transcript for 2011, 2010, 2009 tax year.
2) Letter of current employment.
3) Copy of Bank statement showing Brokerage Account, and saving and Checking Account.
4) Copy of most recent pay stubs.
 
I presume you are applying for yourself, your spouse is the US citizen sponsor. The list looks fine and thorough. Is the medical report sealed and issued recently? Is your US citizen spouse filing I-864 by himself/herself or is there a joint sponsor?

Do you have a birth certificate with parents names and your complete name on it? Is it in English? How about marriage certificate?

The list looks fine and thorough. Is the medical report sealed and issued recently? ITS SEALED and ISSUED 6/7 month ago.

Is your US citizen spouse filing I-864 by himself/herself or is there a joint sponsor? YES ALONE NO JOINT SPONSOR

Do you have a birth certificate with parents names and your complete name on it? Is it in English? YES

How about marriage certificate? GOT MARRIED IN USA SO ITS US FORMAT

Everything looks good, but check the website for information on form type and validity of medical exam, you might be get close to the 12 month validity period..

Special Instructions :
Medical examinations completed on or after January 1, 2012, must be reported using the 10/11/11 version of Form I-693. If a medical examination completed on or after January 1, 2012, is submitted using an outdated form, USCIS will reject the form, and require the applicant to return to the civil surgeon in order to complete and resubmit the current version of Form I-693.​
USCIS requires most applicants seeking to adjust to lawful permanent resident status to submit the results of a medical examination by a designated civil surgeon on Form I-693, which indicates whether an applicant is or is not inadmissible to the United States on public health grounds.​
Note: The results of the medical examination are generally valid for only 12 months. Applicants should schedule the medical examination as close as possible to the time they file for adjustment of status, but provide sufficient time for the performance of laboratory testing or additional testing required under CDC's Technical Instructions.​
 
Perhaps you also want to file I-131 for Advance Parole? It's free if you file it with the I-485. You may not have plans to leave the country but who knows what might come up?

Did you pay the fee for I-765??
