Last entry to USA... how to respond?

My last I-94 is from 1999 on returning from a visit to India. I am on H-1B.
After that, I have visited Canada twice, and as is customary, they never took away my I-94 while leaving USA. On crossing back to the USA on return, they simply waved me through after looking at my passport.
There is no record of my last two entries to the USA. Hence my date of entry (as on I-94) does not match with my actual last entry to USA. In the form 485 I must indicate my last place of entry as Buffalo, NY even though I did not get a new I-94 from there. My last I-94 was issued in 1999 at JFK, New York. Anyone ever got an RFE for this kind of discrepancy in entry date vs I-94 issue date? Any tips for filling out the form 485 in this kind of case? Please let me know. Thanks a lot.

I 94 is your official record of entry and exit to usa. Although it is not customary for INS not to issue a I94 when you cross the border (except when you are driving), you should state your last entry (and inspection) as per your I94.

we had the exact same issue, the lawyer said not to worry
and attached a photocopy of the I-94 we used to have when
we entered from SFO and put in a note saying that we had been
to canada for 3 days and the I-94 was taken off since the
H1 extn I-94 from the I-797 notice had superceeded the airport
\'white\' I-94. I have my greyhound bus ticket stubs to prove that
we actually did travel back to the USA at buffalo, NY...just in case
the INS decides to ask.
