My questions are

1.IS it true that L1 B visa extension can be applied from US only if my I-94 is abt to expire in 3 months.

2.Can my company apply a fresh L1-B while I am in US.

3.Can I visit Canada (for a short trip of 3 days) with only my valid I94 (Visa expired).

You can apply for an extension up to 180 days prior to the expiration date on your I-94.

2.Can my company apply a fresh L1-B while I am in US.

No

3.Can I visit Canada (for a short trip of 3 days) with only my valid I94 (Visa expired).

Yes. But you have to take an appointment with U.S. embassy in Canada and get your visa stamped prior to your re-entry. You are not allowed to re-enter without valid visa.
