My last visa expired in Dec,2003 (from A employer at DC). I got new H1&H4 from B employer from Oct,2003 to March, 2005. No stamping on my passport.
I changed to C emploer and got H1&H4 until June,2005. Now I got visa stamped from C emploer until June,2005.
I don't know exact reasons to receive 221-g. Please check with a good Attorney.

In my case, the old visa in the passport expired on Oct 2003. I have another visa from Jan 2003 thru May 2006, but I changed my company in Jan 2004 and they applied for a H1B which is approved from Jan 2004 thru Oct 2006.
When I sent my documents for the revalidation, I did not send the copy of H1B from Jan 2003 thru May 2006 since they have not asked for all the H1B documents. My guess is that they might have thought that I was out of status from oct 2003 thru Jan 2004.
When required to travel to India, I think I will apply at any American Consulate in Mexico. If they don't grant the visa, then I will be going to India from Mexico and will get stamped. I hope ...........
May I ask you :
Is this your 7th year extension?
have you submitted copies of all H1B ?
do you think any other reason for you?

In my case, my first visa was valid till 2003 October. I changed my company in May 2003, and my second VISA is approved for the period May 2003 - May 2006.
I have sent all the documents including H1-B approval (I-797). I am breaking my head to know where I went wrong or what went wrong in my case.
Can you please update us when you hear back from your attorney.
Also, can you also keep us in loop about your H1-B stamping in Mexico (when you go there). my mail id is spry_me@yahoo.com.
BTW, mine is not 7th year extension.
