I am in H1 status in the US for company A.

Another company B is willing to do premium processing for me .
After i got new H1 from that company , to obtain Visa , should i go to my home country or neednot. This is not H1 transfer ...this is fresh H1

To INS a H1 transfer is like a new H1. But it depends on what company B requests. Let me simplify the logic. Will it say that the candidate is already in US in a valid H1 or is it going to say the candidate is another visa in US (change of status) or the candidate is not in US now?
First case, you do not need to get the stamp immediately.
Second case , you will get h1 status but you have to get the stamp next time you travel outside US
Third case, you have to get the Stamp outside before joining the employer...
