Jaxon/friends.. H1 Revalidation... DS-157 ??

I have to send my H1 B for revalidation to St Louis. Therefore need DS-156 ,157 as per new rules. I downloaded both the forms from http://travel.state.gov/revals.html site.
But, the right hand top corner of DS-157 form says the expiry date as 06/30/2002 and some OMB number, and that's confuses :confused: me because as of date the form is expired or invalid.
Now how can I get a valid DS-157 form or Should I disregard the date and use the same form.

Also, I would aapreciate if you tell me whether it is advisable to send original I797 reciept notice to St. Louis for Visa revalidation.

It is ok

You have to use what is available in the INS web site. Make sure that you take the print out back to back and not in separate sheets.
Yes you have to send the I797 original, make sure that you make a few copies.
If you are prudent, you should send your app with return receipt or use FEDEX.

I think you are right about sending the photocopy still I sent the originals bcoz Jaxon suggested me.
By the way, how much time is required these days to get the passport back after revalidation.
Is there any mean by which you can track the status of your application sent to St. Louis?

Form DS157 Column 12: List Your Last Two Employers
Ques 1: The Last two companies have changed there names and have also relocated. Should he give old names/addresses OR the new ones.

Form DS157 Column 17: List All Educational Institutions Attended ... But Not Elementary Schools.
Ques 2: What is INS definition of "Elementary" - is it Class 1 thru 8?
