Here is the back groud info. From 1988 to 1992 I was on F1, then I applied I-589 ( went thru with finger print )and EAD, I renewed EAD for four year, A # was also isssued it was on all EAD's. At last I got canadian immigration and I withdrew the case and left the country in 1996 and went to Canada. in 1998 job was offered in the usa and I came back on H1B1. I got married her and my wife is also on H1B1. My wife got the labor approvel and she also filed i-140.
now we are filling 1-485. I am applying as derivative.

Question1:In all the form there is a question that give A# IF ANY.
should I provide the(old ) A# even thou that case has been closed.

Question2: beside the Biographic form per my wife's lawer we also have to provide visa history on blank sheet of paper, should I provide from 1988 when I came as student or from 1998 when I entered on fresh H1B1.

1. You will get a new alien number in the 485 receipt. Leave it blank or say None

2. Yes you have to give the complete history. because part of the process is to verify whether you "ever" cheated INS.
