J1 waivered and now offered fellowship, what to do

Hi guys, approved a J1 waiver already, yet to apply for H1B and now has just been offered a Fellowship position.I have one more month to end of residency.Can I go back on my J1 and will ECFMG sponsor.Has anyone ever seen a more complicated case?
Help needed.

This is one of the drawbacks of waiver. If you got waiver the way to go is H or GC. No J anymore
