J1 waiver form which one is correct.

I have been looking for the J1 waiver form and found the NORI posted by New York Consulate general has Biodata sheet where as Washington DC, Indian Embassy has only form and affidavit.

I live in VA hence I have to get my doc attested by DC Embassy. I am confused which one is correct.
Any suggestion?
 
If you have to get attested from DC embassy, I would go ahead with their form only.
Even Chicago consulate also have the same form as DC, which worked well for me.
