I am on a J1 since june 2006 upto june 2009. I took a photopcoy of all the pages of my passport alongwith I-94 in March 2007. Then in june 2007 i visited India and returned in july 2007. i did not take a photocopy of my new I-94 issued during return from india in july 2007. I lost my passport in august 2007 alongwith I-94, DS-2019. i got a new passport made from indian embassy (washington DC) and a new DS-2019. I did not apply for new I-94. Now i am applying for J1 waiver. The form DS-3035 of USCIS asks for I-94 number.
1. Please advice whether i can apply without the I-94 number.
2. Can i use my last I-94 number to apply.
3. Do i have to get a new I-94 number from USCIS before applying.
4. Do i have to get my new passport stamped for applying.
An advice on this issue shall be highly appreciated.
