J1 visa and letter of need

hi...
actually this my first time to log to this wonderful site, i do have two questions that may seem a little bit selly but i have to. i am an IMG looking to do the residency in USA, the first whats the importance of letter of need from my home country and what can i do if they do not issue such letters.
the second question, if i am holding two passport and i am resident of a 3rd country. and i applied to the J1 visa from the one which i am resident in right now. is it compulsary to return to the same country to fullfill the 2 years requirements. or can i stay at any one of those i hold its passport.
i know the 2nd question is alittle bit confusing so am i. please help me to solve these issue.
 
1) Before u get the letter of need u need to get a residency first.once u get the residency u will have to apply to consulate/ Ministry of health along with ur contract to get a letter of need. It is an absolute necessity that u get letter of need as it is one of the requirements of ECFMG J1 program.

2) Whichever pssport u will use for show ur nationality, u will have to use that countries embassy/ dept of health to get the letter of need and that will be considered ur home country to fulfill ur 2 yr HRR.
( some of my friends who are holding indian and canadian pasaport have been using canadian passport for thier J1 visa)
I hope ur questions are answered , most important is to find a position.
