Consider turning depressed and going out on disability. If you have proof of breach of contract by your employer or outright abuse, you might be able to just walk out without having to sit out your notice period.

File bogus complaints about you with the hospital medical staff. They might be bogus, but you will be tied down fighting them and they might hurt you later on in your career.
If you have to work with a hostile employer, make sure that your patient care and documentation are impeccable. Don't slouch off because you know you will be out soon. You don't have to be productive or anything, just make sure that everything you do is 100% perfectly documented.
Shall i tell the state department before the transfer or after it?
Depends on whether they are a physician friendly doh or not. More often than not they are helpful and supportive.
Oh, and before you leave, talk to some local docs that you deem friendly on whether you would be able to give them as a reference in the future. At some point in the future, you might have to provide references for your time spent there, having someone you can call on can be valuable.
