J1 to green card for MD's

I am on J1 visa and will be finishing my fellowship in June 2005 from one of the premier institute in US.
I am required to go to my home country for 2 years before switching to any other visa, eg. H1b.
1) If I go to home country can an employer initiate my green card process while I am outside US and ensure my return to US on green card at the end of 2 years?
2) I also have option of joining VA Hospital part time(3/5) to satisfy my J1 waiver condition and rest 2/5 time working for private group. Can this private group sponser my green card while I am working for them as well as VA?
I am really not interested in spending 5 years in rural US.
Any thoughts!

++++++++++++++++Most probably yes. There are several theoretical problems with this approach, but I have seen it work.
