J1 research to J1 clinical, What is the problem exactly?

For this change to take place, is it the problem of time? I mean too uch time needed to process this change?

Or is it the problem that one should get a WAIVER and how to get this waiver before for J1research, and then apply for J1clinical?

Waiver will prohobit any further J from any type and the only way if you just applied for waiver (approved, denied or pending) is to get another status e.g H or GC for example.
The main problem is that the original J is for certain purpose for example to study molecular biology. If you want to change to another category, including J medical, it should be extension to the study in the same field for example pathology residency. But, if you want to get surgery residency, justification may be difficult unless you prove that during Surgery residency you will be heavily involed in molecular biology of tumors you are going to resect. This proof is not that easy and needs many letters from your current PI, your future residency program director and your personnal statement. If you are going to go to small community hospital, which is mostly the places where FMG go, it is much more harder to justify that connection between your current field and future field whatever you say in the letters. If the clinical J is an axtension and you have the letters and pay the fees, the time is not a problem and in most cases it takes few weeks. Obviously, many cases is outside that range (feww days or stuck forever)
