Is the DOT code decided by the State SWA or the Regional Office?

Hello,

I have a question. I understand that there will be a notice from the State SWA to inform that the State SWA has forwarded the case to the Regional office.

My question is: Does the notice from State SWA have DOT code on it? In another word, is the DOT code decided by the State SWA or the Regional Office?

I believe the SWA puts DOT and prevailing wage for the respective state on the ETA750.

DOT Code is decided by the SWA. When SWA processes your case, they write the DOT code on the bottom right corner of the first page of ETA Form 750-A. You will also notice a date stamp (priority date / SWA receipt date) in the same area (bottom right corner).
