Is I-765 Signature Card required for the I-765 application? What is the form 9003?

Hi Folk,

I have two question about the I-485 application with the I-765.

1. Is the Signature Card required in the I-765 application?
2. What is the form 9003? Is it required for the I-485 application? If yes, where can I get it?

signature card is required with I-765

Form 9003 was discontinued last september and replaced by tax returns(1040) of the most recent years.
The signature card is required according to I-765 filing instruction.
 
Signature card for 765 is not mandatory..

If you do not send the signature card, INS will send the EAD card with a note that fingerprint not available and they might delay the issue of EAD card too. You can walk into the local INS at your convenience and pick up one.
 
IRS Form 9003 is no longer required, as of as of Sept 21, 2001
