Is I-612 form used for J1 waiver through State Health Agency?

I am confused: my waiver petition passed the DOS and is now at the USCIS. On the "online status" site it is classified as "I-612 - waiver of 2 y residence" However, if you look at the I612 form and if you read the instructions, they state:
"This application may be submitted only by an alien who believes that compliance with the foreign residence requirement ... would impose exceptional hardship on his or her spouse or child who is a citizen of the United States or a lawful permanent resident thereof, or by an alien who believes that returning to the country of his or her nationality or last residence would subject him or her to persecution on account of race, religion or political opinion"
So there's nothing for Health Agency or other categories of waiver request...
Am I confused, is this just their way of putting all applicatons in a single boat, or could there be an error on the USCIS side? PLEASE HELP!
 
No. You apply for the state health department sponsored Conrad30 waiver by filing DS3035 (or so) with the DOS visa waiver review division.

On the DS 3035 you apply under 'interested goverment agency', it doesn't have a specific box for state health department. But usually, DOS figures out from your cover letter that you are a physician and lists you on their computer as 'State' case.

I612 is for a hardship waiver which has nothing to do with Conrad30.
