Where do you apply for this?

it is free to order the tax transcripts.

you can call 800-829-1040. listen the instruction carefully. i think you should select the option for "personal account" or something similar.

See IRS link for details. Either call the 800 number or download the form and fax it to the office in your jurisdiction, the fax numbers are part of the instructions for the form. I called the 800 # and got the trancsripts.

A few years ago, you could just walk-in to the local IRS office to get the transcripts as well, I am not sure if they still do that.

Just call the IRS at 800-829-1040 and order the tax transcripts. They are free and arrive approx. 10 days after you ask for them.
