Hi,

One of the questions from the interview is :

Have you used any other names?

I have a nick name that my co workers call me with. I still use my legal name for every thing else, taxes, payroll etc.

Do I need to inform the IO of this when he asks me this question?

if u have a nick name? no why should u he/she isnt ur buddy why would they want to know or care.. only tell them the names u used on documents those r the ones that count not nick names.
 
if ur nick name is used on any document (lease, bank check, driver license, Gas bill, phone bill, electricity bill etc) - then YES - you need to disclose it.

If ur friends just call u that, and it is used on B'day cards, then no need to disclose it.
 
the nick name is even printed on my business cards instead of my official first name.Would this make it as an "official" document.
 
weekly reimbursement checks (not payroll), business cards and my work email.

Do I need 5 year transcripts from the IRS or will the copies of the 1040 work?

heh. ha ha. you must be kidding!! You don't need business card or nickname or your work email.. just stick to legal documents where your real name is mentioned, such as bank statement, birth certificate, your foreign passport, paystubs, driver license/state-issued id card, tax transcripts, bills, marriage certificate if you are married, divorce decree if you are divorced, etc. why should you use nickname as your buddies/friends called you?? It is not important and it is not official. why do you want business card? It is not an official card. It is just like a greeting card. Other name on N-400 is for changing name on purposes (like marriage purposes, perhaps security purposes, court-order purposes, personal purposes, etc.)

Yes, you need the copies of tax transcript (last 5 years)? you can call irs (phone number is on website of www.irs.gov) or fax your request. remember it is not required in the interview, but just play to be safe, just get copies of tax transcripts as well as copies of your 1040 tax returns along with proof of payments (show that you paid or got refunds).

Nickname

I am in your situation. My name is also like yours -- let us say "Samuel" (only an example, my name is something else in another language) being called "Sam" and that is the name I use in the world of business since "Sam" is easy for my American business contacts.

I had chosen to disclose it even for my H1 and GC applications and my lawyer was fine with that in all cases. I think it shows that you have nothing to hide. It is one thing that your wife or husband calls you by one name but it is entirely another when you have a public identity and putting your name on business card does that.
