Got the old passport with the visa?
I would think that should be good enough to prove your age at the time.

Got the passport with the H1B visa stamped plus the entry stamp on the opposite page. Also got a copy of the I-94 too. I have not yet downloaded the form. Should I put the reason for requesting this is as citizenship requisites?

Go to www.sss.gov and then print the form, and apply with PHOTOCOPY of whatever evidence they have asked for. You will get the waiver letter within 3-4 weeks.

Apply today itself.
