Dear Friends,
I have question about the my name that it's consist of 4 parts in my passport:
First name, Father name, Grandfather name, and last name.
When I applied to the lottery I filled my name for the first submitting as well as the forms that I had sent to Kentaky(DS-122 and DS230) as below:
First name, Father name, and Granfather name, and I didn't mention my last name in the passport in the forms because there was only three spaces for full name(First,second, last names).
Is there any issue with that? please advise?
