If anybody knows answers to the following, please reply.

1. Is the CS fee ($675) is individual or for a family?
2. Is it that I have to submit individual forms for each family member or one application would do?

Each *adult* family member will need to submit a separate application and separate application and biometrics fee.
 
Thanks for your replies. Is it past 18 years is an adult? I have two sons aged 14 & 16. So, in that case, do I file one form for me and two sons with a fee of $675.00 and one more for my wife with the fee? Please clarify.

If your children are legal permanent residents they will automatically become US citizens upon your (or your wife's ) naturalization. You will need to fill out one form for you (and pay $675) and one for your wife (pay another $675). Both of you will need to mention your children on the forms but there is no need to fill out separate N-400 forms for them.

However, to obtain documented proof of the children's citizenship, after you become a citizen you will need to file N-600 for them and/or apply for a US passport for them.
 
I sent only one personal check for $1350 for me & my wife. You need to put 1 application for each member and one combines fees in one envelope and send it.
