Infopass appointment question

I am planning to take an Infopass appointment to find out more about my name check.
On page 2, after selecting the country there are 4 options, of which I am confused between these 2 following options

You need Service on a case that has already been filed

You need information or other services

Which one should I select?

If I select this option, next screen is presented with 4 options. Which option should we choose to inquire about application status for the first time.
To me 4th option seems to be correct but I am trying the status for first time and I do not have Service Request ID Number. Please let us know what is the right option to choose.
