iEAD Procedure Wanted

Can anyone please post how to apply for interim EAD after 90 days?
1. What documents to take?
2. Photos?
3. Fees?
4. How to find out which INS to go to?
5. Signature Card?

My area is Fremont, CA.

Here are my 2 cents

1. What documents to take?
a) Photo ID (Passport or California Drivers License)
b) Prepare a I-765 application
b) I-765 Receipt Notice
c) I-485 Receipt Notice (Optional, but better to keep with you)
Thats it!

2. Photos?
Not required.

3. Fees?
No, (make sure that your case is still pending by checking AVM)

4. How to find out which INS to go to?
Check INS.gov for local INS office (I am not sure whether you need to go to SFO INS office or San Jose INS office)

5. Signature Card?
Not Required

Receipt Photo copies will do ?

I have the photo copies of receipts (I-485 and I-765). Is this Ok or I need to have the Original receipts to get the Interim EAD.

They should be fine.

I got mine with copies only, no questions asked for me in San Jose.

Does the iEAD card not have our photos?
