I140 filed- Filing for I485

All,
Filed for I140 in with LC sub earlier this year.Am planning for i485/EAD/AP soon.I had a couple of questions for the same :

1)How important is the "known by other names" column in all the forms? My school,college records do not expand my initial while my passport/visa etc have expanded initial. I should mention the name without the expanded initial? Anyone have experience with this?

2) What is the G28 form for?

1. It doesn't matter if your previous documents have only initials. Apply with expanded initials(full name).
2. G-28 is for attorney representation. This enables attorney to represent your case with USCIS.

In my case also, ssc, intermediate, grduation all in with initials... and passport is in expanded form, I applied both i-140/I485, i guess that wont be any issue.......
 
Here is what I did...

I have my degrees and certs with first + middle + lastname and passport has only first and last name. Attorney suggested to use passport name in the main section and expanded name in the "other names" section so this way I am covered.

For my wife also the attorney put her first+last name in the main area ( as in the passport) and first+maiden middlename+lastname and first+current middlename+last name in the "other names" section

I suggest you let your attorney know all the names and their formats as they appear on all your documents and let him/her take the decision where what fits. They are expert in this regard and try to cover all the loopholes.

Another quick question

Thanks all for reply.While filing 485, do we have to give originals of W2 and 1040 tax return or is a copy sufficient?

Also what about birth certificate?I have a copy of it but my name is not mentioned and I have affidavit from parents stating my name being given later

Send copy of all documents. Never send originals. If they need it - they will ask for it(never heard someone being asked for it except that there is such a message on uscis website).

For Birth Cartificate, if the affidavit bears the stamp of lower court or magistrate - it is fine. DO NOT SEND THE ONE WHERE YOUR NAME IS MISSING - IT WILL CREATE MORE ISSUES.
